ADDRESSING BARRIERS

Addressing Barriers through System of Care

One priority of the Marion-Polk System of Care is to help families with children and youth (ages 0-25 years) access needed care and resources locally. Our Barrier Busting Team was created specifically to look into system-level barriers reported to us by our community.

The Barrier Submission Form is intended to be completed by a community partner. Community partners are those working within a local child, youth, or family-serving organization. Community partners can use the link below to submit information regarding a system-level barrier related to child, youth, and family services within the Marion-Polk Region or on behalf of a community member with whom they are working.

If you are a community member, please work with a local child, youth, or family-serving organization to complete the form. If you are unable to do so and are submitting on your own, reach out to Heather Pascoe-Rosenthal at hpascoe@willamettehealthcouncil.org and we can connect you with someone who can help!

Check out the current status of barriers that have been submitted to the Marion-Polk System of Care!
