The Willamette Health Council (WHC) is seeking OHP members from Marion and Polk County to join its Community Advisory Council (CAC)!
The WHC is the governing body for the Medicaid Coordinated Care Organization (CCO), PacificSource Community Solutions Marion-Polk. We aim to improve the health of those living in our region by using integrated and coordinated care systems to increase quality, reliability, availability, and continuity of care, and to reduce the cost of care.
As a chartered committee of WHC, the CAC will play an integral role in identifying and supporting WHC priorities:
- The CAC will have accountability for managing the CCO Community Health Needs Assessment (CHA) and Community Health Improvement Plan (CHIP); identifying priorities and areas of focus to better serve our region’s CCO members.
- Two members of this committee will represent the CAC on the WHC Board of Directors.
